Portland Bike Library Liability Waiver

Release of Liability and Acceptance of Risk

IN CONSIDERATION of being permitted to participate in the Portland Bike Library I, for myself, my personal representative, assigns, heirs, and next of kin, do:

1. Acknowledge, agree and represent that I understand and know that bicycling is a sport carrying significant risk of personal injury and I know that there are natural and man-made obstacles, hazards, surface and environmental conditions and risks which, in combination with my actions, can cause me severe or fatal injury, I also agree that I, and not the Portland Bike Library or partner organization members, staff, volunteers, contractors, and sponsors, am responsible for my safety while I participate in the Portland Bike Library.
2. Hereby release, discharge, and covenant not to sue the Portland Bike Library, sponsors, organizers, associated entities, and all of their employees, directors, officers or any of the persons connected with the Portland Bike Library, including Quinn Keogh, Aaron Stoertz, and Jake Milligan (Hereinafter the Releasees), from all liability, claims, demands, losses or damages on my account caused in whole or in part by negligence of the Releasees or otherwise.
3. Have participated in a Portland Bike Library orientation and will adhere to safety instruction contained therein while riding a bicycle.
4. Agree to follow traffic laws while participating.
5. Agree that I will be the only one riding the provided bicycle.
6. AGREE TO WEAR AN APPROVED BICYCLE HELMET WHILE PARTICIPATING.
7. Agree that this Release and Waiver shall be valid for and apply to all activities and events connected with the Portland Bike Library during the year in which my signature is dated below.

By typing my printed name and signature below, I acknowledge that I have read this waiver, understand its contents, and agree to be bound by its terms. I understand that I am giving up substantial rights, including my right to sue.
